Camp Application Step 2 – Requirements

Thank you for completing the first step in your MASH camp application. Here are the next steps in the process on being accepted into MASH camp: 
  1. Download the Student Health Assessment Form and the Health Care Provider Health Assessment Form.
  2. Fill out all required information on both forms and upload them here. Once these forms have been uploaded, you will receive an email notification. If you do not receive an email notification, please email NAHEC staff at to inquire regarding application status.
  3. Once the forms are received, NAHEC staff will review them for completeness. These forms will also be reviewed by hospital staff to ensure all information is complete.
  4. Once approved by NAHEC staff and hospital staff, the application process will be complete.

Please note that once the application is complete, it does not mean that you have been accepted into MASH camp. The MASH camp application process is very competitive and only a limited number of students are accepted each year. You will be notified two weeks prior to the beginning of camp via email if you have been accepted into camp.
